We are sorry about that!  If you are unable to submit your application, you first need to double check that every required field has been filled out. 

If you are certain every field has been filled, then one of three issues may be occurring. 

1) Phone Number - This field accepts only numeric values and will not accept any dashes, dots, or slashes. Please check to make sure you are entering your phone number in the following format: ##########

2) State - This field must be submitted as the state abbreviation. You can find this in the drop down menu, or you may type it in yourself. Please check to make sure your state is abbreviated.

3) Insurance - The application requires your insurance be attached in order to submit. You can either drag a picture from your desktop to the icon or click on the icon to upload the image. 

If you are unable to upload the image of your insurance but can upload another image, you may do that.  You will then need to email your updated insurance to insurance@shipt.com so we can associate it with your application!
